22/23 School Year

As an active DCP member you only need to complete the following forms ONCE unless otherwise noted:

 

 

• ODJFS Child Medical Statement (must be submitted each school year)

• ODJFS Child Enrollment and Health Information (unless changes are necessary, you will only need to sign off on the copy already on file. Date for document review TBD)

• ODJFS Request for Administration of Medication (if applicable)

• ODJFS Child Medical-Physical Care Plan (if applicable)

All forms can be submitted in your Jovial Family Portal.
